Dear Society Member,
Your renewal dues are very important and have a
direct impact on your ability to provide quality care. Each members
dues go towards paying for a listing on our website, as well as make
available a discount for the ACLS training at our annual meeting.
Your dues also go towards paying for our delegate seat during the
CMA annual legislative session. We like to remind you that your
renewal helps us keep our CMA membership status and delegate seat,
which is more important at our State level than that of the National
level. Over 150 of your Colleagues are members, so why not renew
now.
Mail-in Application:
Download the CSFPS membership renewal application which is in Adobe's
Acrobat format. Once downloaded, you may print the form
immediately or save it to disk. Complete the paper form and
mail it back us with your payment.
Online Application:
Fill out a simple online application form and then pay for
your membership dues using a major credit card or PayPal account.
